Examining pain related to lateral patella malalignment and focal cartilage overload
It is interesting that many young people with chronic anterior knee pain have either lateral retinacular tightness, distal or lateral patella cartilage softening, or both. Coincidentally, when there is demonstrable lateral tightness and tilt of the patella coincident with lateral patella softening, lateral release often yields pain relief as the lateral articular overload is diminished.

Abstract


Giant cell tumor of bone is a locally aggressive tumor with a high local recurrence rate. Several adjuvant therapies have been employed to reduce the recurrence rate, but their effectiveness remains controversial. The authors attempted local administration of zoledronic acid, a nitrogen-containing bisphosphonate that strongly inhibits bone resorption, as an adjuvant treatment for histologically proven giant cell tumor of bone in 5 patients at their institution. After biopsy, 4 patients were treated with local administration of zoledronic acid with artificial bone and 1 was treated with zoledronic acid without artificial bone. Histologic response to the treatment was evaluated with surgically resected specimens. The 4 patients treated with artificial bone showed local control, with histologic tumor necrosis rates of 90%, 90%, 50%, and 10%. Magnetic resonance imaging showed poor gadolinium enhancement, and histologic examination after local zoledronic acid treatment showed tumor necrosis. One patient without artificial bone showed no histologic tumor necrosis and had local recurrence in soft tissue 18 months after tumor resection. A 3-week waiting period between biopsy and zoledronic acid treatment appears reasonable from the histological study. Complication of this therapy was delayed wound healing and it occurred in 2 cases. Taken together, this case series suggests that local administration of zoledronic acid with artificial bone is a potential adjuvant therapy for giant cell tumor of bone. On the other hand, effective local administration of zoledronic acid requires some bone matrix, including artificial bone. Campanacci's grading is important for predicting the effect of local administration of zoledronic acid. [Orthopedics. 2015; 38(1):e25-e30.].




Resumen



Tumor de células gigantes del hueso es un tumor localmente agresivo con una alta tasa de recurrencia local. Varias terapias adyuvantes se han empleado para reducir la tasa de recurrencia, pero su eficacia sigue siendo controvertido. Los autores intentaron administración local de ácido zoledrónico, un bifosfonato que contiene nitrógeno que inhibe fuertemente la resorción ósea, como tratamiento adyuvante para histológicamente probada tumor de células gigantes del hueso en 5 pacientes en su institución. Después de la biopsia, 4 pacientes fueron tratados con administración local de ácido zoledrónico con el hueso artificial y 1 se trató con ácido zoledrónico sin hueso artificial. Abstract


We report our experience of using a computer navigation system to aid resection of malignant musculoskeletal tumours of the pelvis and limbs and, where appropriate, their subsequent reconstruction. We also highlight circumstances in which navigation should be used with caution. We resected a musculoskeletal tumour from 18 patients (15 male, three female, mean age of 30 years (13 to 75) using commercially available computer navigation software (Orthomap 3D) and assessed its impact on the accuracy of our surgery. Of nine pelvic tumours, three had a biological reconstruction with extracorporeal irradiation, four underwent endoprosthetic replacement (EPR) and two required no bony reconstruction. There were eight tumours of the bones of the limbs. Four diaphyseal tumours underwent biological reconstruction. Two patients with a sarcoma of the proximal femur and two with a sarcoma of the proximal humerus underwent extra-articular resection and, where appropriate, EPR. One soft-tissue sarcoma of the adductor compartment which involved the femur was resected and reconstructed using an EPR. Computer navigation was used to aid reconstruction in eight patients. Histological examination of the resected specimens revealed tumour-free margins in all patients. Post-operative radiographs and CT showed that the resection and reconstruction had been carried out as planned in all patients where navigation was used. In two patients, computer navigation had to be abandoned and the operation was completed under CT and radiological control. The use of computer navigation in musculoskeletal oncology allows accurate identification of the local anatomy and can define the extent of the tumour and proposed resection margins. Furthermore, it helps in reconstruction of limb length, rotation and overall alignment after resection of an appendicular tumour. Cite this article: Bone Joint J 2015;97-B:258-64.

Presentamos nuestra experiencia en el uso de un sistema de navegación por ordenador para ayudar a la resección de tumores musculoesqueléticos malignos de la pelvis y las extremidades y, en su caso, su posterior reconstrucción. También destacamos las circunstancias en las que la navegación se debe utilizar con precaución. Nosotros resecado el tumor músculo-esquelético de 18 pacientes (15 varones, tres mujeres, edad media de 30 años (13-75), utilizando software de navegación equipo disponible en el mercado (Orthomap 3D) y se evaluó su impacto en la exactitud de nuestra cirugía. De nueve tumores pélvicos , tres tenían una reconstrucción biológica con irradiación extracorpórea, cuatro se sometieron a sustitución de endoprótesis (EPR) y dos no requiere la reconstrucción ósea.
